Location: Waukon, Iowa
Start Date: 8/15/2026
Duration: 3 months plus extension
Schedule: Monday through Thursday 7:30 am - 4 pm, Fridays Closed
Estimated Hours per week: 36 hours
Setting: DSO

Requirements
- State License Required: Iowa
- DEA Required Yes
- Certifications Required: Standard
- Minimum Experience: 1 year prefer 2
- Credentialing Timeline: Week before start
