Clinical Magnet, a division of ICON Medical Network, is hiring a dedicated Physical Therapist for a full-time permanent Home Health opportunity covering the Waterbury, Connecticut territory. This position offers the flexibility and independence of home health while being supported by an experienced and collaborative clinical team.

Responsibilities
- Provide skilled physical therapy services to patients in their homes
- Perform evaluations and establish individualized treatment plans
- Assess functional deficits and monitor patient progress
- Coordinate care with physicians and interdisciplinary team members
- Maintain timely and accurate documentation
- Educate patients and families on exercises, mobility, and safety
Qualifications
- Graduate of an accredited Physical Therapy program
- Active Connecticut PT license in good standing
- Minimum of 1 year of experience in a healthcare setting
- Home health experience is a plus
- Reliable transportation, valid driver's license, and auto insurance required
- Ability to travel within the branch coverage area
