Position Overview
The Community Outreach Specialist is responsible for developing referral partnerships, increasing community awareness, and generating qualified enrollments within an assigned territory. This field-based role requires strong networking, consultative selling, and account management skills to build relationships with healthcare professionals, community organizations, and prospective participants.

Key Responsibilities
- Develop and execute territory growth plans to increase awareness and enrollment
- Build and maintain relationships with healthcare providers, hospitals, rehabilitation centers, physician practices, senior housing communities, social service agencies, and community organizations
- Educate prospective participants, families, caregivers, and referral sources regarding available services and eligibility requirements
- Conduct consultations and presentations for individuals interested in learning more about the program
- Maintain a high level of territory activity through outreach, meetings, and relationship management
- Develop a strong pipeline of qualified referral sources and prospective participants
- Meet or exceed established performance metrics related to referrals, enrollments, and sales activities
- Maintain accurate and timely documentation of all sales activities within the CRM system
- Track referral sources, outreach efforts, meetings, enrollments, and follow-up activities
- Manage account records and territory data to ensure accurate pipeline reporting
- Provide regular updates regarding territory performance, referral trends, and goal attainment
- Utilize CRM tools to effectively manage relationships and sales opportunities
Qualifications
- Previous experience in healthcare sales, business development, outreach, account management, or related fields
- Strong interpersonal, presentation, and relationship-building skills
- Demonstrated ability to work independently and manage a territory effectively
- Experience developing referral networks and generating business growth
- Valid driver's license and reliable transportation
- Proficiency with CRM systems and Microsoft Office applications
